    When your doctor’s office makes a referral to Northwest Medical Center, our patient coordinator will collect basic but important information regarding your medical history and reason(s) for seeking help at Northwest Medical Center.
    Appointments
    Patients seeking to be seen at Northwest Medical Center may contact us to schedule an appointment or ask their physician for a referral. Northwest Medical Center is commited to efficient communication with the referring physician regarding treatment plan and progress of our patients.
    Are there forms I need to complete ahead of time?
    Yes, to expedite your first visit, please click on the link above to view and print patient intake forms. Fill them out as completely as you can and bring to your first appointment. There may be additional forms to complete upon arrival.
    Insurance and Billing
    Because Northwest Medical Center believes in equal access to high-quality healthcare, we accept most major insurances. We have reimbursement specialists to assist patients and third party payers with billing concerns.
    Prescription Refills
    You must call the office TWO BUSINESS DAYS before the medication runs out. We do not allow others to call for you.
    After Hours
    For non-life-threatening medical emergencies requiring the immediate assistance of your Northwest Medical Center provider, please call our office and follow recorded instructions. Every effort will be made to assist you in a timely manner.
